Become a new patient before July 1, 2013, and you will have a chance to win a $50 Walmart Gift Card. You can also pick up a referral form at your visit. If you refer a friend, you’ll have two chances to win.

The more friends you refer, the more chances to win. Each time you refer a new patient, you get your name entered into our prize jar. Then, on July 1st, each of our four clinics will have a random drawing, and four lucky winners will be selected.
